Please complete the below Intake Packet and either a Child History (if the client is under 18) or an Adult History (if the client is 18 or older). These documents are fillable PDFs, which can be completed on your computer. The completed and signed forms can either be emailed back to us (info@theCounselingSpecialists.com) or brought with you to your initial appointment.
These three forms are additional documents for your convenience. If you would like us to communicate with an outside party, please complete the Release of Information form. If you would like to change the credit card we have on file, please complete the Credit Card Authorization form. The Privacy Policy document, while attached within the Intake Packet, is listed here for easy access.
